Product Description:

Epoetin alfa is recombinant human erythropoietin, the hormone the kidney secretes in response to low tissue oxygen to drive red cell production in the marrow. In chronic kidney disease the failing kidney stops making enough of it, and the resulting anaemia is a deficiency of the signal rather than of iron or marrow capacity. Supplying the hormone corrects it, and this was among the first recombinant proteins to transform a chronic disease.

It is used for anaemia of chronic kidney disease, chemotherapy-induced anaemia, anaemia related to zidovudine in HIV, and to reduce transfusion requirements around surgery. Two points govern its use. Iron must be adequate, since stimulating the marrow without iron simply exhausts the stores and fails. And targeting a normal haemoglobin rather than a modest one increases death, stroke and thrombosis — which is why it carries a boxed warning and why treatment aims at the lowest level that avoids transfusion, not at normality.
