Product Description:

Ciclesonide is an inhaled and intranasal corticosteroid distinguished by being a prodrug. The administered compound is pharmacologically inactive and is converted to its active metabolite, desisobutyryl-ciclesonide, by esterases present in the airway epithelium. Because that activation occurs preferentially in the lung rather than the mouth and throat, oropharyngeal deposition produces relatively little local corticosteroid effect.

The practical consequence is a lower incidence of oral candidiasis and dysphonia — the local side effects that most often cause patients to abandon inhaled corticosteroid therapy. Systemic bioavailability of the active metabolite is also low, which supports its use where cumulative corticosteroid exposure is a concern.

It is supplied as an inhalation aerosol for asthma at 80 and 160 mcg, and as a nasal spray or aerosol for allergic rhinitis at 37 and 50 mcg.

Sourcing notes. The inhalation and nasal presentations are separate products with separate registrations, and are not interchangeable despite sharing the molecule. Both are device-dependent — a pressurised inhaler or a metered nasal pump — so device documentation forms part of the registration file in most markets, and device lead times frequently exceed those of the formulation itself.
