Cefepime injection is used to treat bacterial infections in many different parts of the body. It belongs to the class of medicines known as cephalosporin antibiotics. It works by killing bacteria or preventing their growth. However, this medicine will not work for colds, flu, or other virus infections.

Cefepime is a fourth-generation parenteral cephalosporin, and the generational label reflects a real difference: it combines the Gram-negative spectrum of third-generation agents, including anti-pseudomonal activity, with Gram-positive cover closer to first-generation cephalosporins. It is also more stable against AmpC beta-lactamases than third-generation agents, which matters against Enterobacter, Citrobacter and Serratia, where third-generation cephalosporins select for resistance during treatment.

It is used in febrile neutropenia, hospital-acquired pneumonia, complicated urinary and intra-abdominal infection, and is supplied as powder for injection, commonly at 500 mg, 1 g and 2 g.

A dosing caution worth carrying in labelling. Cefepime is associated with neurotoxicity — encephalopathy, myoclonus and non-convulsive status epilepticus — particularly in renal impairment where the dose has not been adjusted. It is frequently unrecognised because it presents as confusion in patients who are already unwell.
