Azathioprine belongs to a group of medicines called immunosuppressants. It is used with other medicines to prevent your body from rejecting an organ (such as a kidney, heart, or liver) after a transplant. It is also used in the treatment of rheumatoid arthritis. Azathioprine Tablet works by weakening the body’s immune system, so it does not attack the new organ. The amount of medicine to be taken and how often you need to take it will depend on the type of transplant you have.

Azathioprine is a prodrug of 6-mercaptopurine. Once converted, it is incorporated into DNA as a false purine, blocking the proliferation of rapidly dividing cells — and lymphocytes mounting an immune response are among the most rapidly dividing cells in the body. The suppression is therefore relatively selective for activated immunity. It is among the oldest immunosuppressants still in routine use, and it made organ transplantation practical.

It is used to prevent rejection after kidney, heart and liver transplantation alongside other agents, and in autoimmune disease — rheumatoid arthritis, inflammatory bowel disease, autoimmune hepatitis and vasculitis. The dominant safety issue is pharmacogenetic: patients with low TPMT or NUDT15 enzyme activity accumulate the active metabolite and develop life-threatening marrow suppression on standard doses, so testing before starting is now standard practice. Allopurinol blocks its breakdown and requires a large dose reduction. Long-term use raises the risk of skin cancer and lymphoma.

Off patent, but a licensed oral suspension is a comparatively recent addition and remains originator in several markets — it matters for children and for adults who cannot swallow tablets, replacing unlicensed extemporaneous preparations of uncertain accuracy.
