N(2)-L-alanyl-L-glutamine is a dipeptide that carries glutamine into parenteral nutrition. Free glutamine dissolves poorly and breaks down in solution, while the dipeptide survives heat sterilisation and storage and is split by peptidases into alanine and glutamine after infusion. The product is a 20% solution, 20 g in 100 mL. It is a concentrate, not a ready-to-use infusion: it is hypertonic and must be mixed into a compatible amino acid solution or complete nutrition admixture, never infused on its own.
It is added to the amino acid part of parenteral nutrition for adults in catabolic or hypermetabolic states for a limited period, at a dose capped as a share of total amino acid supply. Trials in critically ill patients have made guidance more cautious, and critical care nutrition guidance advises against parenteral glutamine in unstable patients, especially those with liver or kidney failure.
