Divalproex is a prescription medicine used to treat epilepsy (seizures) and bipolar disorder. It is also used in the prevention of migraine. It helps to decrease the abnormal and excessive activity of the nerve cells and calms the brain.

Divalproex sodium extended-release tablets deliver valproate over roughly a full day from a single dose. Divalproex itself is a stable compound of sodium valproate and valproic acid that dissociates after ingestion; the extended-release matrix then smooths absorption, lowering peak levels and the sedation, tremor and gastrointestinal upset that accompany them, while keeping trough levels high enough for seizure control.

It is used for epilepsy — including generalised, absence and partial seizures — for acute mania, and for migraine prevention. Monitoring is part of treatment: liver function before starting and periodically afterwards, because rare but fatal hepatotoxicity occurs most often in children under two and in mitochondrial disease; full blood count, because of thrombocytopenia; and ammonia if confusion develops. The dominant issue is pregnancy. Valproate is among the most teratogenic drugs in routine use, and regulators in the EU, UK and other markets now require a pregnancy prevention programme, annual specialist review and clear warning labelling wherever it is supplied to women who could become pregnant.

Sourcing notes. Once-daily extended-release tablets are not interchangeable with twice-daily delayed-release tablets at the same total dose; bioavailability differs and conversion requires a dose increase. Packaging must carry the pregnancy warning symbol required in several markets.
