Product Description:

Didanosine is a purine nucleoside reverse transcriptase inhibitor, an analogue of deoxyadenosine that the cell converts to an active triphosphate, which ends the growing viral DNA chain. Stomach acid destroys it, which is why early forms were chewable buffered tablets or powders. The 250 mg and 400 mg capsules on this page are the delayed-release form: enteric-coated beadlets inside a capsule that protect the drug through the stomach and allow one capsule a day, swallowed whole.

WHO guidelines and major regulators have moved to safer regimens, and didanosine is now used rarely, only where it is still registered and a specialist judges it necessary. The 400 mg dose is for adults of 60 kg or more and 250 mg for lighter adults, taken on an empty stomach. Pancreatitis, peripheral neuropathy, lactic acidosis and non-cirrhotic portal hypertension are the major risks. Combination with stavudine is contraindicated, particularly in pregnancy. Allopurinol and ribavirin must not be given with it, and tenofovir disoproxil raises didanosine levels.

Sourcing notes. First confirm that the product still has a valid registration in the destination and that a prescriber has a clear need, since many guidelines no longer list it. Ask for a CoA per batch including assay, related substances and acid-resistance testing of the enteric-coated beadlets, together with the GMP certificate, a CoPP and the dossier. Capsules are packed in bottles with desiccant at room temperature; there is no cold chain, but ask for zone IVb stability data. State strength, quantity and remaining shelf life needed, as low-volume products can be short-dated.
