Clomipramine is an antiobsessional medicine. It is used in adults to treat obsessive-compulsive disorder. It also treats muscular weakness (cataplexy) associated with repeat attacks of sleepiness (narcolepsy) and can manage depression that is unresponsive to alternative treatment.

Clomipramine is a tricyclic antidepressant and the most serotonergic of the class: it inhibits serotonin reuptake strongly, and its main metabolite inhibits noradrenaline reuptake. That serotonergic potency is why it remains the reference treatment for obsessive-compulsive disorder, the condition for which it was the first drug proven effective, and against which SSRIs are still compared.

It is used for obsessive-compulsive disorder, depression, phobias and panic, and for cataplexy in narcolepsy. In OCD the doses are higher and the response slower than in depression, often taking ten to twelve weeks. The sustained-release tablet spreads absorption out, which reduces peak-related nausea and dizziness and allows once-daily dosing. Adverse effects are those of the tricyclics: dry mouth, constipation, sweating, weight gain, sexual dysfunction, postural hypotension and sedation. It lowers the seizure threshold more than most antidepressants, particularly above 250 mg daily, and it is dangerous in overdose because of cardiac conduction effects. It must not be combined with MAO inhibitors.
