Bleomycin is an antineoplastic (anticancer) antibiotic used in the treatment of various cancers, including Hodgkin’s lymphoma, testicular cancer, and squamous cell carcinomas. It works by causing breaks in DNA strands, which prevents cancer cell replication and growth. It is available as a powder for injection (typically 15 units or 30 units per vial) under brand names such as Blenoxane. Product Description:

Bleomycin is a mixture of glycopeptide antibiotics isolated from Streptomyces verticillus. It binds DNA and, with iron and oxygen, generates free radicals that cause single and double strand breaks. Unusually among cytotoxics it causes little myelosuppression, because most tissues contain bleomycin hydrolase which inactivates it — the exceptions being skin and lung, which have little of the enzyme, and which is exactly where its toxicity appears.

It is used in Hodgkin lymphoma as part of ABVD, in testicular germ cell tumours as part of BEP, and for malignant pleural effusion as a sclerosant. Its defining toxicity is pulmonary fibrosis, which is cumulative-dose related, can be fatal, and is worsened by high inspired oxygen during anaesthesia — a fact anaesthetists must be told about for life. Lifetime cumulative dose is tracked carefully, and lung function is monitored. Skin changes and fever are common.
