Parenteral multivitamin and trace element preparations are essential components of comprehensive parenteral nutrition (PN) therapy, providing vitamins and trace elements that cannot be adequately supplied through oral or enteral routes. Commercially available multivitamin formulations in the United States are generally sufficient to meet the nutritional requirements of most patients receiving PN. However, specialized supplementation may be required in certain clinical scenarios. For example, separate parenteral vitamin D preparations, such as cholecalciferol or ergocalciferol, are recommended for patients with vitamin D deficiency who do not respond to oral supplementation.
In addition, carnitine is commercially available and should be routinely added to neonatal PN formulations to support energy metabolism and growth. Choline is another important nutrient that should be included in adult and pediatric PN, though a commercially available parenteral product for routine use is still needed. Current multi-trace element (TE) preparations in the U.S. often require modifications to better match patient-specific requirements. Single-entity trace element products can be used when multi-element formulations are inappropriate, allowing for individualized patient care. Proper inclusion of these vitamins and trace elements in PN helps prevent deficiencies, supports metabolic function, and ensures optimal clinical outcomes for patients receiving long-term or specialized intravenous nutrition.
