This capsule combines Bifidobacterium and Lactobacillus probiotic strains with Oxalobacter formigenes. Oxalobacter formigenes is a gut bacterium that uses oxalate as its energy source, breaking down dietary oxalate in the intestine before it can be absorbed. Lower oxalate absorption means less oxalate excreted in urine, and calcium oxalate is the commonest kidney stone. People who lack colonisation with Oxalobacter — often after repeated antibiotic courses — have been found to have higher urinary oxalate.

Despite the logic, clinical trials of oral Oxalobacter in primary hyperoxaluria did not show a meaningful reduction in urinary oxalate, and there is no established evidence that supplementation prevents kidney stones. The Bifidobacterium and Lactobacillus components are general gut-flora probiotics. Stone prevention remains based on fluid intake, diet, and medication where indicated.

Sourcing notes. Oxalobacter formigenes is a strict anaerobe that is difficult to grow and keep viable, which makes stable supply of live material a serious technical challenge. Buyers should ask for viable counts of each organism at the end of shelf life, not only at manufacture, and for strain identification. Buyers should specify the strains and counts required.
