Cefixime is a third-generation oral cephalosporin with strong activity against Enterobacteriaceae and Haemophilus influenzae, though notably weak activity against Staphylococcus aureus — a limitation that distinguishes it from earlier oral cephalosporins and one that prescribers regularly overlook. Its once-daily dosing and good oral bioavailability made it widely used in urinary tract infection, otitis media, pharyngitis and typhoid fever.

Cefixime has historically been used as single-dose therapy for uncomplicated gonorrhoea, but rising resistance in Neisseria gonorrhoeae has led most guidelines to withdraw that recommendation in favour of ceftriaxone.
